Sangre de Cristo Hospice is looking for a Medical Director to join our dynamic team in our Pueblo, Colorado office. Sangre de Cristo Hospice is the Southern Colorado leader in enhancing the quality of life and maintaining the integrity of individuals and their families in need of supportive or end-of-life services by offering symptom management, support, comfort, and compassion.
The Medical Director is responsible for the oversight of physician services for the Hospice and Palliative Program, in accordance with accepted standards of practice and which support the philosophy, mission and purpose of Hospice. Will also have direct patient care in homecare setting and in our 10 bed inpatient unit with a diverse mixture of urban and rural patients. Will oversee hospital inpatient palliative department and clinic in collaboration with well established oncology practice.
Essential Responsibilities: Clinical oversight of the hospice program; reviews patient eligibility for hospice or palliative services; certifies patients’ condition to ascertain the need for appropriate care. Makes home visits to patients for consultation, assessment/intervention, and for hospice re-certification. Responsible for medical needs of hospice and palliative patient not met by the attending physician. Acts as attending physician for patients referred without a Doctor of Medicine (MD). Consults with attending physicians regarding pain and symptom control and with staff on difficult symptom management issues. Provides regular updates to attending physicians via fax, or e-mail and interacts with Long Term Care facility medical directors and physicians. Participates in community education efforts to promote the hospice philosophy of care and to improve general community and professional understanding of Hospice services. Presents In-services to the medical community as requested and as available. Participates in Interdisciplinary Team Meetings and admission and re-certification decisions.
