Shall be responsible for all aspects of the admission process from inquiries through admission to the nursing home
Responsible for working with and gathering information from applicants’ families, medical staff and/or hospital discharge planners regarding prospective admission to the nursing home.

ESSENTIAL FUNCTION / DUTIES:
Responsible for assisting or leading in all aspects of admission to the nursing home, including, but not limited to: screening telephone inquiries, giving tours of the facility, information gathering and receipt of applications for admission from client and/or family members; verification of eligibility for admission; if Medicare/Insurance Primary coverage is anticipated, gathering and verification of all appropriate information from health care providers for Admissions Team to act upon; pre-admission data entry into the computer system, and; finally, coordination of the admission with Wesleyan staff and completion of final admission papers with the resident and/or responsible party;
Establish and/or maintain a good working relationship with potential referral sources for new admissions such as area hospital discharge planners, physicians and other healthcare providers, and appropriate staff at other Wesleyan facilities, area retirement and assisted living communities;
Visit with potential referral sources for new admissions such as area hospital discharge planners, physicians and other healthcare providers
Facilitate transfers or admissions from other Wesleyan Homes' facilities;
Maintain and track the nursing home 's admissions and waiting lists;
Create reports and miscellaneous correspondence related to admissions and discharges;
Keep current on all aspects of Medicare/Medicaid/Insurance Primary and educate family members as needed;
Maintain compliance with all regulatory requirements, state and federal laws;
Assist clients in understanding these regulations as they pertain to their particular circumstance;
Facilitate Family Council meetings and other programs which promote family relations;
Conduct ongoing resident and family customer satisfaction surveys for improvement of services;
Ensure the accuracy of information entered into our computer system for every new resident’s personal information, responsible party contact information, billing information, and insurance benefit information and initial setup;
Complete and organize new residents’ office files with all applicable information;
One to three years’ experience including hospital, assisted living, skilled nursing or retirement community admissions processes is preferred
Must possess leadership ability.
Must have patience, tact, cheerful disposition, enthusiasm and be able to handle residents and family members based on whatever maturity or cognitive level at which they are currently functioning.
Must be able to meet the public and have the ability to carry out good public relations duties.
Have some knowledge of Medicare and Medicaid programs, various health standards, aspects of the long term care environment, and basic computer skills
Must have the ability to work despite frequent interruptions and with due consideration to residents, visitors, and other staff members
Proficient in computers and computer programs such as word processing, spreadsheets and Microsoft Outlook
Must be able to become credentialed in 6 months from date of hire by referral sources for new admissions such as hospitals, etc.
