This Mountain Home VAMC CBOC provides health care to veterans and entry into the VA health care system. The clinic examines, evaluates, and provides treatment to eligible veterans. Although designated for outpatient services only, referrals are made to an appropriate medical center when specialists or hospitalization is medically indicated. Provides comprehensive outpatient care for a panel of adult patient through a patient aligned care team (PACT) model, utilizing face-to-face, telephone, VA Video Connect appointment modalities for providing care within the standard of care for Internal Medicine/Family Practice patients. Serves as a provider who is highly skilled in the management of a patient's primary care needs or sub-specialty evaluation and treatment. Examines and provides appropriate treatment based on best clinical practice. Orders laboratory and radiology tests and refers patients to specialist when appropriate. Analyzes reports, test results, medical records and examinations to diagnose condition of patients and propose treatment options. Prescribe medications and provides comprehensive instructions for administration Maintains clinical proficiency and familiarity with the Computerized Patient Record System (CPRS), while providing continuity of care for a limited panel of patients.
