The site navigation utilizes arrow, enter, escape, and space bar key commands. Left and right arrows move across top level links and expand / close menus in sub levels. Up and Down arrows will open main level menus and toggle through sub tier links. Enter and space open menus and escape closes them as well. Tab will move on to the next part of the site rather than go through menu items.
The Associate Social Worker is responsible for the development, implementation and evaluation Behavioral Health counseling with individuals and groups. The Associate Social Worker reports to the Director of Behavioral Health.
Conducts comprehensive intake interviews including medical and psychosocial assessment and development of treatment plans for clients in assigned service areas
Provides counseling and case management services for individuals and families including clients with substance abuse and mental health problems
Monitors client needs and service provision and conducts follow-up as needed
Develops and maintains linkages with other health and social service agencies
Develops and facilitates support groups for men, women, teens, parents and/or school-aged children
Conducts and/or participates in assigned family unity meetings and workshops
Provides client advocacy activities including coordination of services for medical and dental care, emotional and practical support, public assistance, community services, and mental health services
Maintains client documentation in compliance with agency protocols
Participates in case conference activities to coordinate care for clients
Prepares written reports as assigned by supervisor
Assists with the preparation of reports to funded sources as needed
