The new CMO-PAC works in a dyad partnership with the Vice President of Post-Acute Care to lead the strategic development and growth of home-based programs and services to improve efficiency, quality, and patient experience in support of acute care transitions and population health initiatives. They will be responsible for developing and driving new workforce and clinical care delivery strategies that improve quality and patient experiences and focus on providing care at Home as the best option whenever possible including expanding home health services, advancing a palliative care strategy, growing in-home wellness services, creating a SNF-at-home model, and working with SSM acute care providers and facilities to ensure a comprehensive approach to post-acute care that reflects and leverages the unique attributes in each market.
The ideal candidate will have experience in a highly complex, multi-hospital healthcare environment with an understanding of the full scope of post-acute care services in an integrated health care system. Candidates must have an earned medical degree and demonstrated experience managing and leading measurable improvements in quality, patient satisfaction and cost of care.
