Our Hospice and Palliative Care Physician role includes: developing and carrying out patient management plans within the context of the interdisciplinary team, providing effective counseling and education to patients and their families to facilitate patient-focused and value driven healthcare and promoting informed decisions about diagnostic and therapeutic interventions based on patient information and preferences. This position requires inpatient consultation, hospice admissions, in-home hospice visits, and inpatient hospice center rounding at Cottage in the Meadow, MYMH's hospice facility that provides a home-like environment and a 24-hour nursing staff. Doctor of Medicine or Osteopathic Medicine (MD or DO) required
Board Certification (or eligible) in Family Medicine or Internal Medicine.
Certificate of Added Qualification in Hospice and Palliative Medicine (or an active candidate for the certification exam as established by the primary specialty board). Alternatively, documented management of 50 palliative care patients within the last four years, with at least 20 encounters in past two years required (hospital or outpatient setting).
Certificate of Added Qualification preferred for Hospice Medical Director.
Washington State Licensure or eligible for Licensure; DEA registered in WA State
Valid driver's license and proof of automobile insurance required
Preferred experience: Understanding of hospice philosophy, plans of care and interventions preferred.
