JOB SUMMARY
The Community Health Worker 2 is responsible for providing cultural mediation between the health care system, social services and the community. This position assists the community in gaining access to needed services and builds individual, community and system capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, patient navigation, follow-up, community health education and information, informal counseling, social support, advocacy and participation in clinical research.
ESSENTIAL FUNCTIONS OF THE ROLE
Conducts needs assessments and develops patient plans. Obtains basic vital signs, measures height and weight and conducts point of care testing.
Navigates patient to appropriate interventions designed to improve access to health care and health status. Provides referral and linkage to follow-up services within the community and within the Organization.
Conducts culturally appropriate skills building self-management education sessions on different topics for groups and individuals.
Teaches basic concepts of health promotion, disease prevention and self-management.
Provides informal leadership to other community health workers including peer observation. Participates in mentorship program, as assigned.
Promotes the provision of culturally competent care by educating staff and others within the health care system about beliefs and practices unique or relevant to specific populations.
Documents plans, goals, education and follow-ups in a timely manner, utilizing agreed upon documentation standards.
Represents program on councils and committees at various levels within the organization. Lead at least one process improvement initiative each year.
