  The Hospice Care Network at Northwell Health  The Hospice Care Network, part of the Northwell Health system, is committed to the highest quality care for those with advanced illness and their families, with services in Nassau, Putnam, Queens, Suffolk, and Westchester counties. Individualized care and support services address physical, emotional, spiritual, and social needs and are provided at all end-of-life stages, including bereavement. Comprehensive hospice services include In-Home Care, Children & Family Bereavement, Inpatient Hospice Care, Pediatric Hospice Programs, and Perinatal Support.
